This form is for the use of Undergraduate and Postgraduate taught students who need to self-certify medical and other personal circumstances resulting in absence from their programme of study for a period of UP TO AND INCLUDING 7 CALENDAR DAYS.
From September 2024 onwards, all absences must be logged through SEAtS, not through the self-certification form. If your programme does not use SEAtS to log attendance or you have any questions about the absences process, then please contact your Student Support team [School Attendance team]
Undergraduates, Taught postgraduates
Student Self-Certification of Absence from Learning is for students who miss learning opportunities or who are unable to engage fully with their studies, due to short-term illness or personal circumstances.
You should submit this form to your School Support Office as soon as you can following your return from absence, or in advance if you are aware of your absence beforehand (e.g in the event of a planned medical procedure). If you have any queries about completion or submission of this form, you should contact your Programme Team/School Support Office for advice. Mitigating circumstances, when unforeseeable or unpreventable circumstances could have, or have had, a significant adverse effect on your academic performance, are covered separately under the Policy on Mitigating Circumstances, and these require the approval of your School. You should discuss this separately with your School. This relates to issues such as extensions to deadlines or absence from assessment.
